Over The Counter Eye Drops - Which Are Right for You?

If your eyes are red and irritated it can be a little overwhelming at the pharmacy trying to determine which of the many over the counter eyedrops available are right for you. Dry eyes and eye allergies are common conditions that are sometimes mistaken for one another. This is largely due to similarity in symptoms, as both conditions can cause irritation, redness, and blurry vision. Compounding the confusion is the fact that an allergic episode can aggravate an already dry eye, exacerbating dry eye symptoms that had been mild and tolerable.
Your best bet is to start with an artificial tear to help lubricate the eye and make it more comfortable. If the source of your eye irritation is a dry corneal surface, this should remedy the problem. However, if the discomfort continues then, at worst, the artificial tears helped flush any allergens from the eye and your next step is to try an allergy eye drop.
Unlike artificial tears, which can be used as often as needed, over the counter allergy eyedrops need to be used as directed for frequency and duration of use. Most over-the-counter allergy eye drops contain topical decongestants, like those found in drops that “get the red out”, which should only be used for a few days at a time. When topical decongestants are used long-term, a medical condition called conjunctivitis medicamentosa can develop. Symptoms include an increase in eye redness and irritation, with more dependence on the eye drop for relief. Spring is in the air! Unfortunately that means allergy season is upon us again. With the welcoming sight of green buds on trees comes the unwelcome increased pollen which means that millions of allergy sufferers will be sneezing, sniffling, and rubbing their red, itchy eyes. If you suffer from seasonal allergies, you may be looking for a way to relieve the discomfort of irritated eyes. Eye allergies are one of the most common disorders of the eye, affecting an estimated 40 million Americans. There are many types of eye allergies but the most common is seasonal allergic conjunctivitis. Approximately 90% of all eye allergy cases are seasonal and usually occurs during the spring and fall. It's triggered by outdoor allergens such as trees, grass and weed pollen. Most of the focusing power in the eye occurs along the front surface of the eye, at the cornea (the clear 'window' in the front of the eyeball). The human eye is usually shaped in a sphere, like a ball, but with astigmatism, the cornea of the eye is curved instead into an oval shape, more like a football. When the eye has these inconsistent curves, light will not travel in and out of the eye normally. The uneven shape of the cornea causes the light entering the eye to scatter in different directions. This can make objects appear blurry and distorted.
In addition to skewed vision, some of the signs or symptoms of astigmatism are:
• Headaches • Eye strain
• Squinting • Difficulty driving at night
Glasses or contact lenses can be prescribed to treat astigmatism, but they generally do not correct the condition. Astigmatism can occur alone as the sole optical error, or may occur together with either myopia or hyperopia. Often, two different lenses will help the eyes focus together, thus offsetting the uneven focusing caused by astigmatism. Astigmatism, however, can be corrected with surgery. Lasik vision correction is the surgery lenses are available that can be used to eliminate astigmatism.
Astigmatism can affect a child's ability to see well in school and during sports but children may be even more unaware of the condition than adults. Ptosis refers to drooping of an upper eyelid of one or both eyes. The droop may be barely noticeable, or the lid can descend over the entire pupil. Ptosis happens in both children and adults, but usually occurs because of aging. Often patients choose to correct ptosis for cosmetic reasons, however in most cases, the drooping upper eyelid results in a reduction in the field of vision. Depending on how severely the lid droops, people with ptosis may have difficulty seeing. Sometimes people tilt their heads back to try to see under the lid or raise their eyebrows repeatedly to try to lift the eyelids. A furrowed forehead or a chin-up head position may indicate that someone is trying to see under their drooping lids. Eyelid drooping can make someone appear sleepy or tired as well as making the eyes fatigue easily, especially when reading.

One of the most common eye problems in older adults is a skin condition called blepharitis . This condition tends to become more noticeable or markedly worsen with the onset of cold weather and drier air conditions. It is often caused by bacteria (usually staphylococcus) or related to a chronic skin condition, such as dandruff, skin allergies, rosacea, or eczema.

Symptoms of blepharitis include a foreign body or burning sensation, excessive tearing, itching, sensitivity to light (photophobia), red and swollen eyelids, redness of the eye, blurred vision, frothy tears, dry eye, or crusting of the eyelashes on awakening. In severe cases, there can be loss of eyelashes or corneal problems. Complications like an eye sty, chalazion, or dry eye syndrome can also be associated with blepharitis.

Blepharitis can be very bothersome and , although it may be very unattractive, it is a common eye condition that usually does not affect vision. In some cases, this condition can become chronic and may never go away completely. Luckily, with good hygiene and the proper treatment, blepharitis can be kept under control.

Treatment of blepharitis involves keeping the lids clean and free of crust. Warm compresses should be applied to the lid to loosen the crusts, followed by a light scrubbing of the eyelid with a cotton swab and a mixture of water and baby shampoo. There are also commercially prepared lid scrubs available over the counter or through your eye doctor. The use of artificial tears or lubricating eyedrops can also offer some relief of the discomfort associated with this condition. If the blepharitis is severe, an eye care professional may also prescribe antibiotics or steroid eyedrops. Because blepharitis rarely goes away completely, most patients must maintain an eyelid hygiene routine for life.

Laser Cataract Surgery Update

The dream of laser cataract surgery is finally here. The first commercial laser was delivered to a doctor in Texas this month. The first real cases should be done shortly. The laser will allow the surgeon to perform 80% of the surgery with the precision of the laser. There are 3 companies competing in the market. LenSx with Alcon, LensAr, and Optimedica. So far LenSx has the early lead with FDA approval. It's still too early to tell who has the best platform. We have learned with other technologies that the first to market isn't always the best. Many doctors feel this could revolutionize cataract surgery the way phacoemulsification did in the 1970s.

Astigmatism Explained

Astigmatism means that your eye is not a perfect sphere but oval shaped like a football. When light enters an eye with astigmatism it is focused irregularly. Astigmatism can be corrected in many ways.

1) Glasses- Spectacle lenses are ground with a cylindrical correction to compensate for the irregular curve in the eye.

2) Contact lenses- Rigid contact lenses rest on the front surface of the eye to provide a new regular surface. Soft toric contacts have astigmatism correction within the lens.

3) Laser vision correction- Lasik surgery will reshape the front surface to correct astigmatism as well as nearsightedness or farsightedness.

4) Cataract surgery- During cataract surgery a toric implant will correct astigmatism. Relaxing incisions in the cornea similar to RK will reshape the cornea to reduce astigmatism.

Astigmatism is usually something we are born with but may shift over time. Sometimes it is caused by corneal disease or trauma. Thankfully there are many ways to treat it.

Droopy eyelids

You know you have them. You try to ignore it. You tell yourself it's just a part of aging.
Droopy lids or more properly upper eyelid ptosis has many causes. Most are related to aging where the skin gets loose and the levator muscle slips back. Some are related to neurolgic problems like stroke, bells palsy or myesthenia gravis. The lowering of the upper brow may also contribute.
The good news is that most of these issues can be resolved with outpatient surgery. Very often they will also be covered by medical insurance.
So stop drooping and get it checked out. All you have to loose is some extra skin.

Laser Cataract Surgery Has Arrived

The hottest buzz at the recent Academy of Ophthalmology meeting (AAO) was the unveiling of Laser assisted cataract surgery. I bet most of you thought it was already done with a laser. Actually current state of the art cataract surgery is done with ultrasound. A technique called phacoemulsification or phaco for short. With the current operation the surgeon manually creates the incision, opens the capsule and then uses the ultrasound to break up and remove the cataract before placing an implant lens.

The laser procedure uses a femtosecond laser, the same laser used to create a flap in LASIK surgery. This laser is capable of making precise incisions into the cornea that are exact every time. The laser will then make an opening in the capsule to a desired size. Lastly the laser energy is directed into the cataract itself to break it's bonds. The only manual parts are the aspiration of the softened cataract and the placement of the implant lens.

The excitement of this new procedure is tempered only by the question of additional cost and who will bear this cost. If current trends continue it will not be covered by insurance and the patient will need to decide if it worth the extra cost.

Glaucoma- The silent thief of sight

Glaucoma has earned the moniker "silent thief of sight" because afflicted patients may have no symptoms until it is too late to save their vision.

The hallmark symptoms of glaucoma are elevated eye pressure and progressive loss of peripheral vision.

1)Elevated eye pressure

Normal eye pressure should be between 10-20. Patients can present to the doctor with pressure as high as 50 with no symptoms, if the pressure has been rising slowly. Elevated pressure stresses the circulation of the optic nerve, slowly damaging the nerve fibers.

2)Loss of peripheral vision

As the optic nerve is damaged it will affect peripheral or side vision before affecting straight ahead vision. A person can lose a significant amount of side vision before they notice it. The good news Glaucoma can be treated if found early. The treatments range from eye drops and laser to surgery for severe cases. If you are over 40 especially if you have a family history of glaucoma you should have a yearly screening exam. Nearsightedness and African descent are also risk factors.

Do yourself a favor and get your eye pressure checked. There may be a silent thief right behind your eyes.

New lens implant options

Implant technology for cataract surgery has improved leaps and bounds in the last 20 years. The evolution started when the first foldable implants were developed. This allowed the implant to be inserted through a 3mm incision. Prior to that the incision needed to be enlarged to 6mm, and needed to be sutured. The foldable implant, made of silicone or acrylic, was the beginning of sutureless surgery. The next big leap was in 1997 with the approval of the first multifocal implant called Array. The multifocal lenses evolved to the ReStor,ReZoom and ReStore 3.0. The Crystalens was also introduced as the only accomodating implant. All of the lenses are designed to give a patient distance and near vision after cataract surgery. Another huge advance is the Toric implant which corrects astigmatism. Just last week it was announced that a lens that corrects distance,near and astigmatism is being used in Europe. With all this new technology it's a great time for patients who need cataract surgery.

Does a Cataract Grow Back?

There is a common misconception that cataracts can grow back after they are removed. I am asked this question every week in the office. I'm here to tell you that they don't grow back. The cataract forms within the natural lens of the eye. During cataract surgery the entire cataract and lens are removed. An implant lens, usually made of acrylic, is placed in the area where the cataract was removed. This lens will stay in the eye for the rest of your life. It can also focus the light so you would not need glasses after surgery.

So why do so many people think that the cataract grows back? The reason is, most patients will later need a laser procedure to clear haze that can form behind the implant. They assume that this is the cataract reforming. It is really the membrane which supports the lens becoming cloudy or hazy over time. It is sometimes called a secondary cataract, which only adds to the confusion. This one minute laser procedure is painless and will clear the vision. The laser only needs to be done once following surgery

What is a Cataract?

There is definitely confusion about what a cataract is. Many people think that it is a film that grows over the eye and can easily be seen. It is actually a clouding or opacification of the natural lens in the eye. The same lens you were born with that has been sitting behind your pupil, now that it is cloudy is called a cataract. This clouding generally occurs gradually due to aging but is also related to UV light exposure, genetic predisposition, trauma and systemic diseases. Some cataracts especially in younger patients will form more rapidly. As a cataract worsens, the vision diminishes and often people complain of glare.
